Principals, Athletic Directors, Coaches, and Parents: It is that time of year again to participate in the annual sports physicals provided by STAR Physical Therapy, & The Bone and Joint Clinic.

We are pleased to inform you that the sports physicals are once again provided at no cost to the student-athlete.
In order for us to make this a smooth process for everyone involved; we ask for your assistance with the following:

1) Please have at least (3) representatives from your given school attend the free sports physicals on their given date. Administrative and Coaches support helps us facilitate a positive experience for your student-athlete.

2) Please advise your athletes to arrive promptly for their assigned times. Student -Athletes that arrive earlier or later, than their assigned time will be unable to participate in the free sports physicals. It is vitally important that we maintain a timely schedule.

3) Recently, the Board of Education has made changes to the requirements of the sports physical. Students that are going to be in the 8th, 10th, 11th, and 12th grades for 2008/09 are allowed to participate in the free sports physicals. Those who do not meet these criteria will be REQUIRED to complete a “WELL CHILD EXAM”. Please visit http://state.tn.us/sbe/. The new “WELL CHILD” form should be available May 1st. 4) Student – Athletes will also be REQUIRED to fill out the HIPPA release form. This can also be found at www.starptusa.com.

5) Please advise the Parent/Guardian that they MUST fill out and sign the EMERGENCY CONTACT & RELEASE found back of the physical form. Please be sure to list ALL contact information. This should include home phone, and cell phones. The physical form MUST be completed PRIOR to receiving the sports physical. ALL student-athletes without completed forms will not be able to participate in the free sports physical.
